Description

Coordinates the practical steps in a patient's care pathway, particularly planned procedures, inpatient progress and discharge preparation. The Case Manager connects patients and families with physicians, nursing teams, diagnostic departments and administrative services so that the next stage of care is arranged without avoidable delay.

Key Responsibilities

  • Review planned admissions and surgical cases to confirm assessments, consents and pre-anaesthesia checks.
  • Explain pre-operative instructions, appointments and care steps to patients and families.
  • Collect information about medical history, medicines, allergies and practical or psychosocial needs.
  • Coordinate with physicians and clinical departments to confirm bookings, investigations and diagnostic results.
  • Track the patient's pathway, identify missing information and raise potential delays with the care team.
  • Support discharge planning and confirm follow-up arrangements and patient understanding.
  • Maintain case notes and communicate changes while respecting patient privacy.

Requirements and Qualifications

A relevant healthcare, nursing, health administration or case-coordination background is appropriate. The role requires the ability to understand clinical instructions, organise follow-up actions and communicate sensitively. Hospital patient-flow, surgical coordination or discharge-planning experience is beneficial. Confidence with electronic records and established patient-care procedures is important.

Skills and Competencies

  • Care coordination
  • patient assessment support
  • pre-operative preparation
  • discharge planning
  • clinical communication
  • appointment coordination
  • documentation
  • confidentiality

Role Expectations

The Case Manager should not make independent medical decisions outside the role's scope. Good case tracking, timely reminders and complete documentation support continuity of care. The post requires prioritisation, careful listening and regular updates to patients and authorised family members when arrangements or care plans change.

The employee should maintain professional standards, follow workplace and information-handling procedures, and cooperate with colleagues to meet service priorities. Tasks should be completed within the approved scope of the position, with questions or exceptions referred to the responsible manager. Dependable attendance, clear communication and a willingness to learn support consistent performance.
