Patient Care Navigator (SKMC)

Job Overview

The Patient Care Navigator at Sheikh Khalifa Medical City supports patients and families by coordinating care, removing access barriers, facilitating referrals and appointments, and maintaining communication across the multidisciplinary healthcare team. The position combines patient advocacy, nursing practice, care coordination, education and service improvement.

Key Responsibilities

• Assess patient and family needs, identify barriers to care and develop individualized plans to support timely and coordinated treatment.
• Act as a primary contact and patient advocate, providing emotional support and helping patients navigate healthcare services and resources.
• Coordinate referrals, specialist appointments, diagnostics, procedures and other elements of the patient pathway.
• Maintain communication between patients, families, physicians, nurses, social workers, case managers and other healthcare providers.
• Conduct follow-up calls after discharge, monitor progress and escalate medication, pain, appointment or care-related concerns to the appropriate provider.
• Ensure required investigations and results are available for consultations and coordinate services across SEHA and external facilities when necessary.
• Use EMR systems and specialty databases to maintain accurate patient information and support continuity of care.
• Provide patient and family education, health-promotion information and guidance on self-management and available support resources.
• Monitor documentation, patient-safety and infection-control requirements and participate in quality-improvement initiatives.
• Support staff education, service-development projects, multidisciplinary meetings and patient-experience improvement activities.

Requirements

• Bachelor's degree in Nursing or an equivalent qualification from an accredited program with at least three years of study.
• Valid/current professional nursing license.
• BLS certification required; ACLS and PALS are desirable.
• Charge Nurse experience plus additional experience in line with DOH PQR, or equivalent staff-nurse experience as specified by SEHA.
• Care coordination or specialty certification is desirable.
• Strong communication, customer-service, clinical coordination and organizational skills; Arabic is desirable.
