Description
We are seeking a compassionate and experienced Care Coordination Nurse to provide age-appropriate care while serving as a key leader within the clinical team. This role balances patient advocacy, interdisciplinary collaboration, education, and operational leadership to support SEHA’s mission and maintain the highest standards of nursing care in alignment with Department of Health (DoH) regulations.
🎯 Key Responsibilities
Leadership & Team Development
- Lead and mentor nursing teams within assigned clinics or hospitals
- Support clinical governance and staff development through structured mentoring and performance oversight
- Promote collaboration across interdisciplinary care teams and optimize resource utilization
- Supervise staff performance, budget alignment, and adherence to nursing scope and SEHA policies
Patient-Centered Care
- Deliver evidence-based, culturally sensitive care aligned with DoH standards
- Conduct assessments, develop care plans, monitor patient progress, and advocate for patient needs
- Provide emotional support and ensure safety and dignity across all patient interactions
Health Education & Clinical Training
- Educate patients, families, and clinical staff on disease management, preventive care, and health promotion
- Actively support SEHA health education initiatives at facility and community levels
Quality & Safety Oversight
- Uphold SEHA’s Code of Conduct and promote patient satisfaction
- Manage patient feedback and concerns to facilitate continuous improvement
- Maintain clinical excellence in communication and customer service
Coordination & Communication
- Serve as a liaison among healthcare departments to support seamless care delivery
- Ensure compliance with regulatory targets and facilitate efficient flow of information across teams
📋 Qualifications
Required Experience- Charge Nurse role + 2 years (as per DoH PQR)
- OR Staff Nurse role + 4 years (as per DoH PQR)
- OR Current SEHA nurses who have completed SEHA specialty training in lieu of care coordination experience
- Experience in a tertiary healthcare setting
- Bachelor’s degree in Nursing from an accredited program (minimum of 3 years of study)
- Preferred: Certification in Care Coordination and/or specialty-specific qualifications
