Description
The Charge Nurse – Care Coordination is responsible for delivering patient-centered, age-appropriate nursing care within the defined Scope of Practice by the Department of Health (DOH). This leadership role integrates clinical expertise, care coordination, staff supervision, patient advocacy, and health education to ensure high-quality healthcare delivery. The Charge Nurse upholds SEHA’s mission, vision, values, and professional nursing ethics while promoting health and enhancing patient experience.
Key Responsibilities
Leadership & Management- Provide clinical leadership and mentorship to nursing staff within the care coordination unit.
- Monitor and evaluate team performance, ensure staffing efficiency, and support professional development of team members.
- Support interdisciplinary collaboration across departments and contribute to resource planning and budget management.
- Enforce adherence to clinical standards, policies, and regulatory requirements in line with SEHA and DOH guidelines.
- Assess and address physical, psychological, and social needs of patients through comprehensive nursing care plans.
- Coordinate and monitor delivery of safe, evidence-based, and culturally sensitive care.
- Facilitate patient education, emotional support, and discharge planning.
- Act as a liaison between patients, families, and healthcare providers to promote seamless transitions of care.
- Educate patients, families, and caregivers on health conditions, post-discharge care, and self-management.
- Deliver ongoing staff training on clinical protocols, patient safety, and quality improvement practices.
- Promote and lead health education campaigns internally and within the community.
- Champion high standards of safety, infection control, and incident reporting.
- Maintain patient confidentiality and ethical practice aligned with the Professional Nurses Code of Conduct.
- Support data collection and analysis for performance improvement, patient satisfaction, and clinical audits.
- Facilitate multidisciplinary meetings and promote effective communication among healthcare providers.
- Ensure timely documentation of care processes, discharge plans, and referrals in electronic health records (EHR).
- Collaborate with external agencies as needed for continuity of care and patient resource coordination.
Qualifications
Minimum Educational Requirements- Bachelor’s Degree in Nursing or equivalent from an accredited program with a minimum of 3 years of study.
- Care Coordination Certification
- Specialty Certification in relevant clinical or leadership area
Experience Requirements
RequiredOne of the following:- Current Charge Nurse experience + 2 years relevant experience per DOH PQR
OR - Staff Nurse experience per DOH PQR + 4 years of clinical experience
OR - Current SEHA Staff Nurse or Charge Nurse who has completed SEHA Specialty Training Program (no prior Care Coordination experience needed – only applicable to existing SEHA employees)
- Experience in a tertiary healthcare facility, particularly in women’s and newborn services
