Description

The Case Coordinator plays a vital role in facilitating coordinated, patient-focused care under the supervision of the Senior Case Coordinator. This entry-level position supports interdisciplinary collaboration and ensures timely progression through the continuum of care while aligning with clinical standards, insurance protocols, and SEHA’s commitment to excellence.

🔑 Key Responsibilities

Managerial & Care Facilitation

  • Coordinate multidisciplinary teams and accredited healthcare providers to support quality patient outcomes
  • Develop care plans covering placement, discharge planning, utilization management, and transition support (upon reaching competency)
  • Facilitate documentation compliance for medical records and insurance reimbursement
  • Act as a bridge between physicians and finance regarding cost-effective clinical care

Technical Coordination

  • Apply evidence-based screening criteria for admissions and continued stay reviews (under supervision)
  • Collaborate with nursing, physicians, and allied health staff to maintain seamless patient flow
  • Identify obstacles to discharge and recommend interventions
  • Understand insurance benefits to optimize treatment coverage within authorized limits
  • Support the collection and review of medical documentation and patient progress
  • Engage in educational, transitional, and financial counseling upon competency

Quality & Safety

  • Participate in clinical performance audits, reporting on length of stay, resource use, and readmission rates
  • Assist in quality assurance initiatives and contribute to continuous improvement efforts
  • Support physician documentation processes related to medical necessity and coding

Patient & Family Education

  • Assist in planning and delivering health education, psychosocial support, and community resource referrals
  • Update learning resources in alignment with case management practices
  • Document educational efforts and outcomes accurately

📋 Qualifications

Required Experience
  • Charge Nurse: Per DoH PQR + minimum 2 years in case management
  • OR Staff Nurse: Per DoH PQR + minimum 4 years in case management
  • OR Current SEHA Nurse (Senior/Charge/Staff) who completed SEHA Case Management Training Program (experience waiver applicable only for SEHA employees)
